Something like that is planned for the future. It's a long way off yet at this point, though.

If an item has been completely removed (its vnum has disappeared), there's nothing we can do but fail to load it. It's then lost forever when the character is saved.

That's not *totally* true. Second attack and third attack are completely separate skills. Third attack has a much lower percentage chance to hit than second attack does. If you practice third attack without practicing second attack, you'll only ever get two hits. plus you'll get the second less ofte...
